Immunization Requirements for School Attendance in Idaho (Immunization requirements have been updated for the 2011-2012 school year, with some new required immunizations. Be sure to check your child’s immunization record to the new requirements to ensure that he/she is up-to-date with the requirements.)
Idaho Certificate of Immunization Exemption Form (This state form is newly updated for 2011-2012. This version of the form is required for Immunization Exemption – other or previous forms are no longer acceptable.)
